If it only happens when braking, it's probably a warped rotor.

well I pulled the drivers side wheel off and it looks like my rotor has a grove in it.. Im assuming that means its warped??

It means it's worn, that's all. Your vibration could also be pad build up, corroded caliper pins, sticking caliper piston...

Since we have no idea of year, model, mileage or maitenance for your rig I would suggest getting discs and pads and take your time at each wheel. Replace your disc, clean up your caliper pics, inspect your calipers and move to the other side.
