Occasionally drops out of DRIVE when stopping can pull on back on shifter and works

Sorry for the terrible title. I have an 06 Supercharged with 70k on it. The truck drives/shifts just fine (I do have a funny hum sometimes at low rpm but dont think its related) but occasionally it will slip out of D coming to a stop. If i slight pull back on the shifter towards D it works just fine again. If I shift to neutral and back to D then usually it will work just fine the rest of the journey. Sometimes I even get it going around a corner. RPM's will raise and I notice I'm out of gear. Slightly pull back and its ok.. If this happens enough I'll get Transmission Fault but it goes away next key cycle. Is there something on the linkage under the trim that needs adjusted? maybe its just dirty?

Any help appreciated!
