This past weekend while off roading near Lake Arrowhead a Coolant Hose Failed. It actually got close enough to the radiator fan to cause the damage. The hose was original. Not sure if the heat, movement of engine etc.. caused the the movement of the hose. Regardless I replaced with an aftermarket hose which is shorter and keeps the hose farther away from the fan. I zip tied as well.

Thanks, jgibson. Perhaps there was a missing tie-back or shield for that hose?

And OF COURSE it waited to fail until you were in the mountains. My $1,200 alternator replacement strategically waited until I was at Lake Isabella before failing, a town in the low mountains. :)
