When do you replace brakes (mm's)?

I have read that brakes are usually replaced at 2 mm's. Is this typically correct?

Also, are new brakes considered 12 of 12 mm? My front brakes are 6 mm and my rears are 9 mm so I am trying to guage how far along I am.

Those numbers seem on the high side - I had thought the OEM pads are 5mm. THe sensors supposedly break between 2-3mm leaving you with some lead time. In my mostly urban driving I get about 20-25k miles if that helps.
